Submit a Ticket

Use the connected Google Form to log an incident, service request, or change request with the NovaTech support team.

Complete the required fields and submit one request per issue. Do not enter passwords or confidential information in the form.

Create and publish your required Google Form, copy its responder and embed links, then paste them into js/google-form-config.js. The form will appear here after you save the file and refresh this page.

After setup, responses will be recorded in the Google Sheet linked to the form. The optional Apps Script automation in this project can generate ticket IDs and send email acknowledgements.

Before you submit

Log one main issue per ticket.
Include the affected device or Asset ID where known.
Describe impact, urgency, and any error message.
Do not include passwords or sensitive personal data.

Support-level response targets

Level 1Within 2 hours

Level 2Within 24 hours

Level 3Within 48 hours*

These are response-time targets from the Task 1 SOP, not guaranteed resolution times. Level 3 may take longer for highly complex cases. The support team assigns priority after assessing impact and urgency.

How tickets are handled

The support team logs and triages the request, assigns priority P1–P4, attempts Level 1 resolution, escalates if required, communicates progress, and closes the ticket after resolution confirmation.
